If you believe your Lively Benefit Access Card has been lost or stolen, you can quickly cancel and replace it through your Lively dashboard.
From the main menu, navigate to Debit Cards. Next to the affected card, select the three dots and choose Cancel Card. You’ll be prompted to select a cancellation reason and then confirm by choosing Cancel and replace. A replacement card will be issued and mailed to the address on file, typically arriving within 10–13 business days.
Please note: If you have already been issued three cards for yourself or your dependents, a $5 fee will apply for each additional card. This fee will be debited from your linked bank account.
