How do I order FSA debit cards for my dependents?

From your Lively FSA dashboard, navigate to the menu and select “Debit cards” then “Order debit card.” If you have not yet added dependents to your FSA, you will be prompted to do so first.

Please note that if you have already been issued 2 FSA debit cards for yourself or your dependents, you will be charged a $5 fee for each additional debit card. The $5 fee will be debited from your linked bank account.
