You can order additional cards for dependents ages 18 and older from the Debit Cards section of your Lively account. If you have not yet added dependents to your account, you’ll be prompted to do so before placing the order.
Please note: If you have already been issued three cards for yourself or your dependents, a $5 fee will apply for each additional card. This fee will be debited from your linked bank account.
