You can use your Lively Benefit Access Visa® Card to pay directly for eligible expenses covered under your plan(s) or pay out-of-pocket and submit a reimbursement request through your Lively dashboard.
When using your card, some purchases are automatically approved (auto-substantiated), while others may require you to provide documentation such as an itemized receipt or Explanation of Benefits (EOB).
Note: Substantiation is the IRS-required verification process to confirm that benefit funds are used for qualified medical expenses. It may involve submitting a receipt, EOB, or other documentation.
Automatic substantiation occurs at select merchants whose systems comply with IRS requirements through an Inventory Information Approval System (IIAS).

IIAS-certified merchants: These merchants automatically identify eligible products, so no additional documentation is required.

90% stores: At stores where at least 90% of merchandise is FSA- or HRA-eligible, you may need to upload a receipt or EOB to verify the purchase.

Non-certified or local merchants: Purchases at non-certified stores will need to be paid out-of-pocket. You can then submit a claim for reimbursement through your Lively dashboard.

Healthcare providers: You can typically use your card directly at healthcare offices such as doctors, dentists, or labs. Upload your EOB and receipt for substantiation.
If a purchase is made at a non-qualified merchant, pay out-of-pocket and submit a reimbursement claim through your Lively account.
