Enrollment files help streamline employee setup, automate updates, and ensure accurate benefit enrollment data. While Lively provides a standard file format for employers, we can also process other file designs if that is preferred by your administrator or benefits provider. Depending on your provider, using your existing file format may reduce fees and shorten implementation time.

Enrollment File Overview
This section introduces the foundational elements of creating and formatting your Lively enrollment file. It explains the accepted file types, and highlights the importance of matching each column header exactly as shown in Lively’s file guide. Following this structure ensures your file uploads correctly and employee data processes without error.
Employers can also download pre-formatted templates directly from the "Upload Files" section of the Employer Dashboard. These templates include sample rows for enrolling employees, assigning plan codes, and completing actions such as adding or terminating participants. Reviewing these templates helps confirm that your file includes the correct plan codes and formatting for your account.
Enrollment File Training
This video walks through the steps of preparing, uploading, and reviewing your enrollment file, as well as how to resolve any errors that may appear. Before submitting your file, make sure all required termination fields are complete, including the employee’s termination date, personal email address (especially if their account uses a work email), and termination reason (such as employment terminated or benefits terminated).
Once your file is finalized, upload it through the "Upload Files" section of your Employer Dashboard. If any errors are detected, you’ll be able to download a detailed error report showing the exact rows and reasons for each issue. After correcting the errors, simply re-upload the file for processing.
Understanding File Plans
This section provides an overview of how to correctly structure and reference plan information within your Lively enrollment file. It explains how the "Plan Code" column is used to ensure each employee is enrolled in the correct benefit plan and emphasizes the importance of using exact plan codes as listed in Lively’s file guide.

Enrollment File Actions
This section explains how to use the "Action" column in your enrollment file to manage employee information and benefit plans accurately. Each row of your file corresponds to a specific action, and the correct action type ensures employee data and plan enrollments are processed properly.
Primary action types:
Plan Enroll: Used to enroll employees into a specific plan. Each row can only enroll one employee in one plan, so multiple plans require multiple rows.
User Update: Allows changes to employee details after enrollment, such as name, phone number, address, or payroll schedule.
Plan Update: Used to modify existing plan details for employees already enrolled, rather than starting or ending a plan.
User Term: Ends an employee’s account and terminates all active employer-sponsored plans at once.
Plan Term: Ends a specific plan without removing the employee entirely. Each plan termination must be listed on a separate row.
