Employers cannot order or replace debit cards on behalf of employees. Employees can manage their own card requests directly through their account dashboard without needing to escalate the task to you.
Lively provides debit cards for HSA, FSA, Standard HRA, LSA, and Commuter accounts. Please note that debit cards are not currently available for MTA, and certain HRA plans.
You can direct your employees to follow these steps to manage their cards:
Log in to their Lively account.
Select "Debit Cards" from the menu.
From this page, they can order a primary card for themselves or a dependent, or report a card as lost or stolen to initiate a replacement.
Important: Please encourage employees to verify that their mailing address is up to date in their "Account Settings" before requesting a new card to ensure successful delivery.
