What You’ll Need

A third-party receipt (on company letterhead or signed by the provider)
– Provider/merchant name
– Dependent’s name
– Description of care
– Dates of care (must be within your plan year and already incurred)

Optional: You can also utilize Lively's Smart Claim Submission feature. Simply download & complete the DCFSA Claim Form and upload it while filing your claim.
How to Submit a Claim

Visit secure.livelyme.com/login and log in.

From your Dashboard, click Get reimbursed (top right).

Follow the claim submission steps:

Enter your expense details.

Upload your receipt (and claim form, if using).
