To add a beneficiary to your HSA:
-
Log in to your Lively dashboard.
-
Open the menu (three horizontal lines in the top left) and select “Account Settings.”
-
From the tab options, choose “Beneficiaries.”
-
Follow the prompts to enter the required beneficiary information.
You can designate both primary and contingent beneficiaries for your HSA.
-
A primary beneficiary is the first person or entity who will inherit your HSA funds in the event of your passing.
-
Second or contingent beneficiaries are those who will inherit your HSA funds if your primary beneficiary is unable to do so.
You may also list a trust as either a primary or contingent beneficiary. To do so, you’ll need to provide the trust’s Tax Identification Number (TIN). Most U.S. trusts have an assigned TIN; however, in some cases, a living trust may use the grantor’s Social Security Number (SSN) instead.
If you’re unsure how to record your trust or designate beneficiaries appropriately, please consult a tax professional for personalized guidance.
