Your Lively Post-Deductible Health Reimbursement Arrangement (PDHRA) funds can only be accessed once you’ve met your plan’s Out-of-Pocket Requirement, the amount you must spend toward your medical insurance deductible before your HRA becomes available. This amount is determined by your employer.
After meeting the Out-of-Pocket Requirement, you’ll activate your PDHRA by submitting an Explanation of Benefits (EOB) through your Lively account. This EOB serves as confirmation that the threshold has been met and allows Lively to make your funds available. Your EOB summary must be provided by your health insurance provider and must include:

The total out-of-pocket amount spent to date, and

Confirmation that this amount is equal to or greater than your plan’s out-of-pocket threshold.
To activate your PDHRA:

Log in to your Lively online account at secure.livelyme.com/login.

On your dashboard, click “Activate” in the message displayed.

Follow the guided prompts to upload your EOB and complete the activation process.

Lively will review your submission within 5 business days and notify you once it’s approved.
Once your PDHRA is activated, you can use your funds to pay for eligible medical expenses as of the date of service associated to the EOB submitted for activation. Your HRA funds remain available until they are depleted or the plan year ends.
