The funds in your Lively Post-Deductible HRA (PDHRA) can only be accessed after you have spent a certain amount of funds towards your medical insurance deductible. This amount is known as the Out-of-Pocket Requirement of the PDHRA and is determined by your employer.
Once the Out-of-Pocket Requirement amount has been met, you must activate your PDHRA by submitting an Explanation of Benefits (EOB) document through your Lively account online. This document shows that you met the threshold and is required in order to activate your PDHRA and thus access the funds in the account.
Acceptable EOB documentation
Lively will require a single EOB Summary provided by your health insurance provider that contains the following information:
- Display the out-of- pocket amount spent to date
- The out-of- pocket amount displayed must be equal to or greater than the out-of-pocket requirement threshold
Learn more about EOBs here.
