Below are common reasons why your Lively Benefit Access debit card may have been declined.

The card was swiped as a “Debit” instead of “Credit”
- You entered a PIN at the point of transaction, your debit card does not have an associated PIN
- Swiping instead of inserting your card’s chip may result in decline, be sure to pay by inserting the chip

Some or all of your purchase items are not eligible (check what’s eligible). Bag fees, cash back, charitable donations, etc., should not be included in the same transaction

Your purchase amount exceeds your available balance

The information you entered does not match what is on file (security code, zip code, or expiration date)

The merchant is not categorized as a healthcare retailer/provider or is not IIAS-certified (read about the types of merchants)

Your plan has ended or is suspended

Your debit card is not activated
Log in to your Lively account to review your plan details or any actions that need your attention. Remember, if you paid for eligible items out-of-pocket, you can submit a claim for reimbursements from your Lively account.
