If you suspect your debit card has been lost or stolen, you can cancel it by logging into your dashboard, navigating to “Debit cards” from the menu, and clicking “Report lost/stolen” next to the appropriate card. You will have an opportunity to order a replacement card at that time.
Please note that if you have already been issued 2* FSA debit cards for yourself or your dependents, you will be charged a $5 fee for each additional debit card. The $5 fee will be debited from your linked bank account.
*Effective 1/1/2025, Lively will increase the limit to issue up to 3 cards at no cost.
